I'm Molly Milgrom. I work with people who are competent, deeply feeling, and deeply exhausted — the ones everyone else calls when things get hard, who rarely call anyone themselves.

Most of the people I see aren't in crisis. Their lives look fine, sometimes good, from the outside. But they're anxious and sad while still performing in every area they're expected to, and they've reached a point where the way they've always managed is starting to cost too much.

  • Are curious, motivated, and willing to stay with the work even when it's slow

I work well with people who want depth and are willing to look at how their history, relationships, and unconscious patterns are showing up in their present-day struggles — not people looking for a quick fix.

When This May Not Be the Right Fit
  • I may not be the best fit if your primary treatment needs center on ADHD, Autism Spectrum, or Borderline Personality Disorder. If that's your situation, I'm happy to help you determine whether another clinician may better match what you're looking for.
  • I don't provide crisis or acute psychiatric care.
  • I don't see clients on a court-mandated basis.
  • I see individuals and groups, not couples.
  • I'm private pay only and don't accept insurance.
  • I work with adults only.
  • If you want a short-term, skills-only, or protocol-based approach, this likely isn't the right match — my work is depth and relationship-oriented, and it tends to take longer to land.

How I Work
My approach is integrative and relational.

The lens I bring to everything:
  • Integrative psychodynamic — Whatever you bring in — anxiety, dissatisfaction, relationship difficulty — I pay close attention to how your early experiences shaped the rules you learned about feeling and expressing emotion, and the patterns you repeat without noticing, including in the room with me. Most of what gets called "symptoms" is rooted in relational history, so we use the therapeutic relationship itself as a place to learn about your patterns and rework what isn't working.
  • Strengths-based — Alongside whatever's difficult, I pay just as much attention to what's already working for you, and use your existing capabilities as material for change rather than starting from what's broken.
  • Systems perspective — I don't look at your struggles in isolation from the rest of your life. I use a bio-psycho-social-spiritual lens that accounts for power, culture, race, religion, gender, ability, socioeconomic status, and other layers of context that shape what you're carrying.

Methods I bring in depending on what you're working on:

  • When anxiety, trauma, or emotion live primarily in the body rather than in words, I sometimes use sensorimotor psychotherapy to help access experiences that insight and talk alone don't reach.
  • When it's useful to work directly on specific thoughts, behaviors, or immediate next steps, I bring in CBT and solution-focused techniques to shift what's maintaining the emotional state.
  • In my therapy groups specifically, I work from a contemporary/modern analytic orientation, targeting long-standing patterns of self-attack and freeing up the energy that gets tied up in them, with a strong focus on the here-and-now and on improving how you communicate emotionally.

Sessions modulate between supportive and challenging. They're collaborative but not tightly directive — you set the agenda, and I help guide us toward what you actually came in for. I'm active in the room with you.

For some clients, this work happens one-on-one. For others, it also includes a therapy group, where the same relational patterns show up in real time with other people who can reflect them back in a way individual work can't.

Why Clients and Colleagues Choose Me
I've been doing this work since 2011, licensed since 2015, and running my private practice since 2015. Alongside my clinical training, I bring my own lived experience with difficult family dynamics, living abroad, high anxiety, disconnection from myself, and career change — I know what it's like to feel lost, anxious, and confused while wanting something more, and I know that it's possible to feel different.

Colleagues who refer to me describe me as smart, deeply caring, and willing to challenge people. The pattern I see most often: colleagues refer their own clients' friends and family — people they specifically want to land with someone they trust.

Every person's experience is different, and I can't promise specific outcomes. Many clients who are a strong fit begin noticing meaningful changes within the first few months, especially in anxiety, self-understanding, and relationships, while deeper patterns continue to unfold over time. Many choose to add a therapy group at that point, where work on shame, depression, and loneliness tends to accelerate.
